We recommend that you immediately contact your primary care physician to assist you in choosing a new health care provider.  If you have an emergency before the transfer of your care to another provider, you should dial 911 or go to the nearest hospital’s Emergency Department.

Upon your request we will make your medical records available to your new health care provider.  You will need to complete a release of information, before we can transfer those records to your new physician.  Once completed, the authorization should be faxed to (541) 681-8587 or mailed to 445 Harlow Rd #200, Springfield, OR, 97477.

Fax .   In the meantime, your records will remain on file at our office.
